What is a Care Coordinator?
A Care Coordinator is a nurse trained to coordinate the care of asthma and obesity patients. Care Coordinators monitor patient care and make interventions for patients not meeting targeted goals through:
-Establishing a plan of care according to the patients needs
-Provide information that can be used to empower the patient to manage their condition
-Increase health screening with the goal of early detection and treatment of illness and disease
-Improve the care and treatment of patients with chronic health conditions
-Reliable medical care
Our care coordinator will provide your child with additional medical support and coordinate their care under the supervision of your childs physician. This may include scheduling appointments when needed, outreach and telephone follow-up, education, coaching and other support to assist your child in managing his/her medical condition.
